If you want stand-alone UniCare dental insurance, use the UniCare dental application form. DON'T use the UniCare health insurance application form, and DON'T use the UniCare online application.

UniCare PPO dental insurance is available to all Illinois residents. However, people who live in areas with a concentration of participating PPO dentists are better able to obtain maximum value from the plan. So we recommend this plan for residents of the following Illinois counties:
