UniCare Dental Plan Availability

UniCare dental PPO coverage for individuals and families can be purchased with your UniCare medical coverage, or it can be purchased "stand-alone."

If you want stand-alone UniCare dental insurance, use the UniCare dental application form.  DON'T use the UniCare health insurance application form, and DON'T use the UniCare online application.
