MedPlan Access is committed to protecting your privacy. In providing services as a state-licensed insurance agent to consumers in Illinois and in Indiana, we ask you to provide us with personal information. We only use this personal information so we can provide our services to you.
We don't sell, trade or give away personal information provided by Illinois consumers and by and Indiana consumers. In addition, we use industry leading technologies to ensure the security and confidentiality of the personal information you provide us. Our privacy policy applies to personal information such as your name, phone number, street address, email address, and health history.
Obtaining Personal and Non-Personal Info

This site contains secure forms which you can complete to obtain health insurance quotes, or to instruct us how and where to deliver insurance application documents. - Insurance Applications
Our customers send us completed health insurance applications, containing personally identifiable information, which we forward to the insurance company at issue. The handling of this information is further discussed in the following "Limited Disclosure of Personal Information" section. - Log Files
Like most web sites, we gather certain information automatically and store it in log files. This information includes internet protocol (IP) address, browser type, internet service provider (ISP), referring search engine, operating system, and the time and duration of a visit.We use this information, which does not identify individual users, to analyze trends, to track users' movements around the site and to gather statistics about our user base as a whole. To offer the products described in our site, MedPlan Access contracts with insurance organizations and HSA trustees or administrators. If you submit an application or sign up for these products, we'll share the information you have provided as necessary for the organization to provide their service.These insurance companies, HSA trustees and HSA administrators use your personal information to process your application or enrollment form. They are legally obligated to maintain strict confidentiality. Their communications with you will be via email, phone, fax or postal mail. - Residents of States other than Illinois and Indiana
This website contains an instant health insurance quote feature utilizing the software and website of Norvax Inc. This feature provides health insurance quotes for persons residing in Illinois and in Indiana, which are the states where we maintain insurance licenses.In using this feature, Illinois or Indiana residents are NOT required to input any personally identifiable information.However, MedPlan Access is not licensed to market insurance products to residents of states other than Illinois and Indiana. As a consequence, when residents of these other states use the instant quote feature, the information they provide may be forwarded to insurance agents licensed in these states. - Legal Obligations
We may report your personal information when compelled by law to cooperate with regulators or law enforcement authorities.
Co-Branded Pages and Partner Sites
Much of this site is devoted to specific products offered by insurance organizations with which MedPlan Access maintains an independent contractor relationship. You can recognize this content by the insurance company names and brand logos appearing prominently on the applicable site pages.
To assist our visitors in gathering health insurance information, obtaining quotes and applying for health insurance products, we provide link connections to sites of many of our trusted partner companies such as UniCare, Blue Cross Blue Shield of Illinois and Anthem Blue Cross ad Blue Shield. Personally identifiable information provided on these sites will be used by these organizations to provide quotes and process applications. You Control the Information You Provide
MedPlan Access endeavors to collect only the minimal personally identifiable information required to do our job for you. If you'd prefer that we not use certain personally identifiable information, we'll honor your request. However, this may mean we can't deliver the service you've requested, such as providing an accurate health insurance quote or submitting a health insurance application.
In your interactions with our contracted partners, such as the health insurance companies we represent, we'll assist in communicating your information use preferences to the organization at issue. However, these companies are bound by their own privacy practice statements.

Your Control Over Communications
We may use your personal information to contact you when providing our services. For example, during the application process we may email you to inform you of application status or to obtain additional information needed to continue processing of the application.
