Forms

This site contains secure forms which you can complete to obtain health insurance quotes, or to instruct us how and where to deliver insurance application documents.
Insurance Applications

Service Providers such as Insurers and HSA Trustees

To offer the products described in our site, MedPlan Access contracts with insurance organizations and HSA trustees or administrators. If you submit an application or sign up for these products, we will share the information you have provided as necessary for the organization to provide their service.

These insurance companies, HSA trustees and HSA administrators use your personal information to process your application or enrollment form. They are legally obligated to maintain strict confidentiality. Their communications with you will be via email, phone, fax or postal mail.

We may use your personal information to contact you when providing our services. For example, during the application process we may email you to inform you of application status or to obtain additional information needed to continue processing of the application.
