HumanaOne Eligibility Requirements

In Indiana, Humana will issue new HumanaOne individual health plans for applicants age two months to 64.5 years. The maximum age for a dependent child is 25 years if the child is a full-time student and 24 years if the child is not a full-time student.  Dependents may include step-children or legally adapted children.

Children-Only Coverage

Children can be insured alone.  The custodial parent or legal guardian who can attest to child's health history must complete the application.  If an underwriting interview is required, the person that completed the application will be interviewed.  The youngest child will be the primary applicant, and any others will be listed as dependents.  Newborn applicants require an eight week well baby exam, with normal results.

Through HSA-compatible health insurance plans such as the HumanaOne products are available on a "child only" or "children only" basis, a child claimed as a dependent on a federal tax return cannot be the owner of a Health Savings Account (HSA) -- even if the child is insured under his or her own HSA-compatible high deductible health insurance plan.

Other Health Insurance

Humana doesn't allow dual coverage.  If you're applying for HumanaOne coverage and you're currently covered by another plan, you must replace that coverage with HumanaOne.  Don't terminate existing coverage until you've been notified that Humana has agreed to insure you.

U.S. Citizenship

If you're not a U.S. citizen, to be eligible for HumanaOne you must have lived in the U.S. for a minimum of two years and have consulted, within the last two years, a physician in the U.S.
