Traditional PPO Overview

Humana One traditional PPO coverage is simply designed.  The basic plan has four deductible options:
  • $500 deductible
  • $1000 deductible
  • $2500 deductible
  • $5000 deductible

With each of these options the deductible is waived for preventive care.  Prescription coverage is subject to a $500 deductible, through you can upgrade (additional cost) your Humana One benefits to a $0 prescription deductible.

The complete listing of optional benefits for the Humana One plan offered in Illinois and Indiana includes:
  • $0 prescription deductible
  • Office visit co-pay (max 4 visits per year)
  • Routine maternity coverage (no benefit waiting period for Indiana residents)
  • Term life insurance
  • Comprehensive dental plan (Illinois only)

Offered in at least 26 states, Humana One is a good choice for people who want to be able to move to a different state while maintaining the same health plan.

Rate Guarantee

All the Humana One plans are subject to an initial 12-month rate guarantee.
