Anthem 360° Health is comprised of support services, specialized resources and health management tools for Anthem members. 360° Health components include:
ComplexCare identifies members with multiple or complex health issues, then proactively addresses health concerns before they become more serious.
Members have their own dedicated nurse care managers who helps coordinate care, offers self-management strategies and serves as an information resource. ComplexCare can help members make more informed choices about their care and treatment options, relying less on high-cost acute medical services.
As an Anthem member, you can establish your own personal health page at MyHealth@Anthem. The personalized content you can access includes news on topics you select. You can also provide online updates to your Anthem health insurance account information and sign up for online health newsletters.
Anthem's ConditionCare program identifies, engages and manages members who are living with chronic conditions such as asthma, diabetes, heart failure, chronic obstructive pulmonary disease (COPD) and coronary artery disease.
A dedicated nurse and support team of dietitians, exercise physiologists, pharmacists and health educators work to help Anthem members better understand their risk factors, their physicians care plan and how to improve their health.
If you need help determining whether a problem requires medical attention, Anthem's 24/7 NurseLine provides around the clock direct assistance from registered nurses.
Future Moms offers prenatal education and support for mothers-to-be. OB-GYN nurses, joined by a team of obstetric specialists, coordinate individualized care for expectant moms.
Each future mom receives a prenatal kit and a birth kit, complete with child-health and safety information. After delivery, Anthem nurses are available for follow-up, to provide information about common issues such as post-partum depression and other conditions which contribute to increased care and costs.
