Anthem Eligibility Guidelines
Applicants who meet the following guidelines are eligible to apply for Anthem Blue Cross and Blue Shield Lumenos Indiana health insurance coverage:
- Cannot be eligible for Medicare or Medicaid
- Must not engage in hazardous activities, (i.e. underwater diving, racing, rodeo activities, professional sports, etc.) Any exceptions must be approved by the Anthem Underwriting Department.
- Must be between the ages of "newborn" and age 64. Newborns (whose parents/siblings are not current Anthem policyholders) are eligible for individual coverage only when released to home under routine care.
- Must be a permanent resident of the state of Indiana
- Must be a legal U.S. resident
- Not applying for coverage while traveling abroad
- Not currently pregnant or an expectant parent
- Anthem Blue Access coverage will not be provided when other Major Medical insurance is to remain in force.
- Domestic Partners are eligible family members (though Anthem may require completion of an affidavit).
- Applications may be submitted for children only (not including a parent), if all children are under age 18.
- Dependent children can be covered to the end of the month in which they turn age 25.
Dependent Child Age Limitation
A covered dependent child who loses eligibility upon reaching the maximum age may apply for his/her own Anthem coverage. If the application is received with 31 days of losing eligibility, acceptance to the same plan, or similar plan (if the original plan is no longer offered, with no lapse in coverage is guaranteed. Pre-existing condition credit and credit for any deductible met under the original plan will be applied to the new plan.
